1. Dates/Terms/Eligibility:
The "LOT Groups - At your service" incentive begins for groups booked between January 1 - May 31, 2012 and September 1, 2012 – April 30, 2013. Groups must depart from the US or Canada between dates stated. Tickets for group travel must be issued in the U.S. or Canada by an IATA agent, between January 1 – May 31, 2012. The incentive is open only to US or Canadian travel agents with a valid IATA number in eligible states/provinces (with the exception of Quebec). Void where prohibited.
2. How to participate:
Travel Agents must book their groups via the LOT group request form available on LOTtravelagents.com. Agents must include the code, LOGRP11, in the comment section of the group request form. Group requests made via LOTtravelagents.com that do not include the promotional code, or that are requested by methods other than LOTtravelagents.com, will not be eligible for promotional incentives. The group sales team will keep a record of all group transactions. The group desk will forward a confirmation of award once reached and all terms/conditions of the group contract have been met. Agent must acknowledge incentive and sign a form for LOT’s record to complete award fulfillment.

4. Incentive distribution:
Incentive awards will be distributed after the each group level has been achieved. In order to achieve each level, the group must have departed. (For example: 1 group booking for 19 passengers departing Feb. 10, 2012 would receive an incentive of $100 after Feb. 10, 2012). After the 3rd group booking, the incentive levels begin again at the 1 group level. This can be repeated for group bookings throughout the incentive period January 1 – May 31, 2012 and September 1, 2012 – April 30, 2013.
A W-9 form (Request for Taxpayer Identification Number and Certification) will be forwarded to awardees for completion and a 1099 form will be prepared and forwarded to the awardees at year's end or any incentive valued over $600. If an awardee is from Canada, The Travel Agency or individual agent may be asked to complete a W-8BEN form in order to receive their incentive. All other expenses not specified herein with respect to the above incentive are the sole responsibility of the awardees.
If travel agency does not allow personal awards it must be noted in a letter so that the award may be given directly to the Travel Agency. Please allow 2 - 4 weeks for processing.
5. Conditions:
Eligible groups are defined as 10 or more passengers traveling together on LOT coded and operated transatlantic flights on inbound and outbound sectors. Meeting, incentive and series groups / movements are not eligible for this promotion. Large groups that are requested in multiple, smaller groups in order to obtain additional incentives will be eligible for a single qualifying incentive. Meeting, incentive and series groups / movements are not eligible for this promotion. Eligible tickets must be validated on LOT Polish Airlines (080). Incentives are non-transferable, non-refundable and non-changeable. Cash (other than promotional award levels) or credit alternatives will not be offered. LOT Polish Airlines reserves the right to substitute incentive of similar value should the specified incentive become unavailable. Incentive awardees must be a valid IATA agent in good standing with LOT Polish Airlines and at least 18 years of age and a legal U.S. or Canada resident. Incentive awardees that chose a travel agent ticket will be issued a letter with instructions and terms regarding such ticket. Incentive awardees are responsible for all required travel documents (i.e.: valid passports, visas, inoculations, etc.) Awardees must make reservations and request tickets at least 30 days in advance of preferred departure date, but no more than 90 days in advance. Seats are capacity controlled and subject to availability at the time of booking. Tickets are valid 3 months from the date of issue. Blackout dates for flights originating in the United States are June 1, 2012 through September 15, 2012 and December 15, 2012 through January 7, 2013. All travel must be completed by May 31, 2013. LOT Polish Airlines condition of carriage shall apply to all air transportation Incentive tickets are non-transferable, non-refundable and non-changeable. Cash or credit alternatives will not be offered. LOT reserves the right to cancel or amend the terms of this incentive without notice in the unlikely event of major catastrophe, war, earthquake, or any actual, anticipated or alleged breach of any applicable law or regulation or any other similar event or to provide substitute incentive of similar value should the specified incentive become unavailable.
By participating in this incentive, the awardees grant the promoter’s permission to use their name and likeness for advertising and future incentive purposes without additional compensation (except where prohibited). Awardees are liable for any tax liability accruing as a result of this incentive. This incentive is void where prohibited. Online entrants must have valid e-mail address and it is the entrant’s responsibility to update LOT of any change in their e-mail address. No transfer, assignment or substitution of an incentive is permitted by any awardees. Entrants agree to be bound by the terms of these Official Rules and by the decisions of LOT, which are final and binding on all matters pertaining to this incentive.
By participating in this incentive, entrants agree to release and hold harmless LOT, its officers, directors, employees, and agents (the "Released Parties") from and against any claim or cause of action arising out of participation in this incentive, or receipt or use of any incentive, including, but not limited to: (a) unauthorized human intervention in the incentive; (b) technical errors related to computers, servers, providers, or telephone or network lines; (c) printing errors; (d) lost, late, postage-due, misdirected, or undeliverable mail; (e) errors in the administration of the incentive or the processing of entries; or (f) injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant's participation in the incentive or acceptance, receipt use or misuse of any incentive. Entrant further agrees that in any cause of action, the Released Parties' liability will be limited to the cost of entering and participating in the incentive, and in no event shall the Released Parties be liable for attorney's fees. Entrant waives the right to claim any damages whatsoever, including, but not limited to, punitive, incidental, consequential, direct, or indirect damages.
By entering this incentive, entrants acknowledge that they have read and understood and agree to be bound by these terms and conditions
